INSURANCE DISCLAIMER
Bio-identical hormone replacement is a unique medical practice and is considered a form of preventative medicine. In most cases, insurance does not recognize it as necessary medicine but considers it like esthetic medicine. Therefore, expenses are usually not covered even though the physicians and nurses are board-certified, trained specialists. Our office will not bill your insurance company for your pellets and pellet insertion procedure.
We can provide you with an itemized receipt showing the amount you paid out of pocket on the day of your pellet insertion. If you choose, you can use this receipt to submit a claim with your insurance company. This receipt includes applicable CPT and ICD10 diagnosis codes for your pellets and your pellet insertion procedure. Sending this receipt to your insurance company is your responsibility.
We cannot guarantee payment of any amount, although some patients have received partial reimbursement from their insurance companies for pellet and/or pelleting procedures.
It is your responsibility to communicate with your insurance company. We kindly request that you do not ask your insurance company to call us. Our office will not communicate in any way with your insurance company in reference to pellet and pelleting procedure claims generated outside our office. (We bill insurance companies for lab and office visits only.)
